Patient Profile
The Patient Profile is the central hub for managing all information related to a specific patient. It provides a comprehensive view of their clinical history, consultations, documents, and personal details.
Patient Header & Highlights
The top of the profile provides immediate access to high-level information and global actions:
- Quick Demographics: View the patient's full name, gender/birth sex, date of birth, and care modality.
- Clinical Highlights:
- Consultations: Total count of sessions recorded.
- Monthly Average: Frequency of visits over the recent months.
- Medication Adherence: A quick indicator of the patient's adherence status.
- Meeting Link: Direct access to virtual consultation links when available.
- Actions:
- Download Profile: Export a comprehensive overview of the patient's clinical and personal data.
- Manage Patient: Access the menu to delete the patient record (requires appropriate permissions).
Overview
The Overview tab provides a detailed snapshot of the patient's clinical background and personal demographics.
Clinical Background
- Chief Complaint: The primary reason for the patient's visit.
- Chief Complaint History: Context and background regarding the current complaint.
- Diagnosis: Recorded clinical diagnoses for the patient.
Medical History & Allergies
- Medications: Detailed tracking of both Current and Previous medications, categorized by Psychotropic and Other medications.
- Allergies: A list of known allergies with a dedicated section for clinical notes.
Physical Exercises
Monitors the patient's lifestyle habits:
- Regularity: Indicator if the patient exercises regularly.
- Usage Metrics: Tracking of days per week and hours per day spent on exercise.
- Types: Specific exercise categories (e.g., Aerobic, Strength, Flexibility).
Referral & Personal Info
- Referral Information: Records how the patient arrived at the clinic, supporting both Internal (colleague referral) and External sources.
- Personal Information: Vital demographics including Birth Sex, Nationality, Email, and Mobile Number.
Substances
The Substances page allows for detailed tracking of a patient's substance use, coffee consumption, and tobacco history.
Substance Use
Monitor the use of various substances:
- Substances: A list of specific substances used by the patient.
- Notes: Clinical observations or details regarding each substance's usage.
Coffee Use
Detailed tracking of daily consumption patterns for:
- Coffee: Number of caffeinated beverages per day.
- Sodas: Daily intake of carbonated soft drinks.
- Tea: Daily tea consumption.
Tobacco History
A comprehensive record of smoking and tobacco use:
- Smoker Status: Tracks both current and past smoking status.
- Product Tracking: Specific products used (e.g., cigarettes, cigars, vaping).
- Usage Metrics:
- Packs per day: Quantification of usage.
- Years smoked: Total duration of use.
- Quit Date: Recorded for past smokers.
Consultations
Track and manage all clinical encounters with the patient:
- Previous Consultations: A chronological list of past sessions showing the date, modality, and progress notes.
- Add Consultation: Create new consultation records directly from the profile.
- Management: View detailed session overviews or remove records as needed.
Documents
Securely store and manage files related to the patient's care:
- Files List: View all uploaded documents (e.g., PDF reports) associated with the patient.
- Uploads: Switch to upload mode to add new clinical reports or intake forms.
- Management: Download files for local review or permanently delete obsolete records.
